Please fill out the “Financial Aid” form to apply for financial aid. Specific sections of this form are automatically filled by Lonestar Soccer; please do not alter these sections as it will cause a delay in the processing of your application. Please fill out the following sections of the 4506-T ONLY: 1A and 1B, 2A and 2B (if filing jointly), 3, and 4. Please sign where applicable, and do not fill out any other fields.

Only ONE application per family is required, regardless of the number of children you have in the club.

 

The deadline for 2009-2010 Financial Aid applications is not published at this time.
