T-SHIRT Make-Up Order Page

Instructions for completing the make-up orders for those pre-orders

that were not picked up or were not filled correctly/completely.

The form which can be found on the link below must be completed

with all information requested.  This includes the reference information

for the order as it was emailed to the responsible party.  The order and

transaction number are in this list as well as the dollar amount of the

original order.  All orders not fulfilled during the tournament or during

check-in are entitled to pick two additional shirts - that is the last

section on the form.  Please be sure to complete the shipping info

at the bottom of the form so we can have these delivered to the

correct location - this does not have to be the primary contact.
